The story of that appendage si unravelled in one of her comics, just google after Spunky Knight. I think the japanese manga´s name is Pink Phaia, in English is something like Spunky Knight XXX (yea, Eros did it)
After reading Alice, I admit I dont trust their translations anymore, but…

Phaia Rouge, the (S)Punky Knight, had too much sex with the owner of the weapons shop in volume 4 of the original manga story line.
The owner was a special woman(?), and it made some obvious changes in Phaia’s body.

Her extra large clitoris was surically changed into a penis and a Cowpers Gland (Wikipedia) added, which causes the large bulge.
She was getting a bulge before that, because she was pregnant, again, which was shown in a small insert in the previous series.
What do you expect, when a nymphomaniac works as a prostitute to make ends meet?
I have a hunch that her being “on the pill” is only in the Eros Comix translation.

Even if that were the case (which it isn’t, as Anona pointed out), testicles don’t form above your pelvis. They form closer to below your spinal cord, near your ass (the inside, not the outside), and “descend” above the perineum (“taint”). Which is why when they get cold, they “shrink” back in that direction. THE MORE YOU KNOW.
@Anona: That wikipedia article is full of wrong information. Though I honestly don’t even know if it’s worth correcting, as it seems dangerously at risk for deletion. It’s probably only through sheer laziness or apathy to that article’s existence that it hasn’t been deleted already.

@Anonymouse: It turned out that I used the Wikipedia site for the links it had, since it really didn’t have much in the way of facts, other then the author’s name and the names of the series that Eros Comix did.
I was hoping that DesuDesu would supply the URL to the scanlations for the earlier ones to those who want the leadup, but I don’t feel it’s appropriate for me to post it in here.
Eros Comix only has a few volumes (chapters) of those earlier stories for sale at $1. Apparently they are closing them out. OR maybe compiling them into one big book. OR redoing them in the original front to back layout (right to left) rather then the flipped format they came out in.

There you go. The whole shebang. All of it more recently rescanned, much cleaner, and much larger resolution than the scans from the days of old (well, except issue 3 of the original series, and 1 of Extreme). Mostly by bubbadg, and 2 and 3 by MaxBigFoot. Both of them have topics going on the Erotic Rapture forums for rescans of classic series like that.
And just to clarify, most of this series (and most of the classic Eros stuff in general) is out of print in english, and Eros haven’t reprinted (or printed anything new for that matter) in a few years now. They’re more or less dead, probably as a result of their translation/editting partner, Studio Proteus, going belly up.

For me it would have been about 6 years ago when I first read it. Probably my first hentai manga, and issue 4 was definitely my first encounter with futa. And not just any futa… double-dicked futa with paizuri. Good times…
The re-scans definitely brought back good memories. And while the series definitely got weirder, and his art style went in a direction I wasn’t crazy about, the Extreme and XXX series are still worth a read.

Delfas modified Phaia’s nipples at the same time as her clit.
He replaced them with far more sensitive tissue that gets extremely erect, and apparently will ejaculate when milk is being produced.
Delfas was changing her to sell as a sex slave, but didn’t get the brain changes done before she escaped.
